Location: our office is conveniently located in Indian Shores, on Gulf Blvd. just south of the Pub Restaurant in the La Concha Plaza, 20001 Gulf Blvd., Suite 7.

Financial Considerations: This practice does not accept insurance. We will fill out a HCFA-1500 form for your insurance company. Fees for services are due at the time the treatment is provided. Payment options are cash or major credit card.
Appointments: We are open with and without appointments. Patients are taken in order of the severity of their medical or surgical condition. We guarantee same day appointments. If possible we ask that you call in advance so that we may reserve adequate time for you. We will make every effort to schedule your appointments as compatibly with your personal schedule as possible. In consideration of all our patients, a minimum of twenty-four hours notice of change is required.
